Role is enforced. Facility is enforced on reads, not on writes.
These routers require an authenticated caller in doctor, nurse or admin
— pharmacists additionally for the formulary — and check the facility
when it arrives in the path or query string. They do not check it when
it arrives in a JSON body, which is how every admission, appointment and
formulary write carries it. A caller can therefore write into a
facility that is not their own.

Role.ADMIN passes the facility check
unconditionally, in every direction.
