Either nothing has been recorded for that filter, or this deployment
has no audit sink wired and the trail is being kept nowhere. Those are
different problems; the count above says which.
Where the entries come from
The trail carries identifiers and never names: patient_id
and actor_id, the fields touched, the stated purpose
and the outcome. That is deliberate — an audit log holding patient
names would be a second copy of the record with none of its
protections.
Reading this screen is itself an administrator action against
audit.read. Nobody browses it anonymously.
