Consent
One record per purposeEach box below becomes its own consent record with its own purpose. Withdrawing one later does not withdraw the others.
The medical record number is allotted by CBS on save — the next in this facility's series. It is not typed here.
What the approved design draws that has no field yet
DK-01 as approved also carries a preferred language, an attendant
or next-of-kin block, an insurance tab, and a claim code that
links the patient's own Care app. None of those has a column
behind it: RegisterPatientRequest accepts name, date
of birth, sex, phone, email, address, blood group, external
identifiers and consent, and nothing else.
They are listed here rather than drawn, because a field that silently discards what a clerk types is worse than a field that is honestly absent — and the insurance details in particular would be typed once at admission and never seen again.
